TOWN OF WAKE FOREST
WAKE FOREST DOWNTOWN IMPROVEMENTS PHASE III

Sealed proposals are invited and will be received by the offices of the Town of Wake Forest Planning Department at the 3rd Floor of the Wake Forest Town Hall, 301 South Brooks Street, Wake Forest, NC 27587, until 2:00 P.M., Thursday, December 13, 2018, at which time at a meeting in the Wake Forest Town Hall 2nd Floor Board Room, 301 Brooks Street, Wake Forest, North Carolina 27587, the sealed proposals will be publicly opened for construction consisting of furnishing and installing the following:

Approximately 1700 linear feet of street construction of White Street, between Holding Avenue and Elm Avenue, approximately 300 linear feet of street construction on East Owen Avenue and approximately 200 linear feet of street construction on Brooks Street. The work includes grading, paver installations, landscaping, storm drainage, paving, curb and gutter, sidewalk, signing, traffic control, pavement markings, utilities, erosion control, and other related items as shown on the plans.

Hard copies of all forms are required for the bid submittal. Copies of the plans, contract documents, and specifications may be obtained from the Accent Imaging website (www.accentimaging.com) at the cost specified by Accent Imaging. Plans must be purchased via the website for bidders to be registered on the official bidders list. Only bids from bidders on the official bidders list will be considered by the Town.

A mandatory pre-bid conference for all prospective bidders will be held at 2:00 P.M. on Thursday November 29, 2018 301 Brooks Street, Wake Forest, North Carolina. Also any questions may be directed to Jason Pace by email at jason.pace@kimley-horn.com until 5:00 p.m. on Thursday, December 6, 2018; after which time no further questions will be entertained.

No proposal will be considered or accepted unless at the time of its filing, the same shall be accompanied by a cash deposit, cashier's check, or certified check on a bank or trust company insured by the FDIC and authorized to do business in North Carolina in an amount equal to five percent (5%) of the proposal. In the alternative, a five percent (5%) bid bond issued by a corporate surety licensed by the State of North Carolina may be filed with the proposal. Bidders must be properly licensed under Chapter 87 of the General Statues of North Carolina and must comply with nondiscrimination provisions. The Town is an equal opportunity Municipality/Owner and invites small and minority contractors to bid. The Town reserves the right to reject any or all proposals.

Mailed bids should be sent to Town of Wake Forest Planning Department; Attention: Candace Davis; 301 Brooks Street, Wake Forest, North Carolina 27587. The proposal form provided by the Town of Wake Forest shall be used and shall not be taken apart or altered. All bids must be in a sealed envelope clearly stamped or marked (in letters of 1/4" or more),

Request for Qualifications # 19-001
Wake Forest Parks, Recreation and Cultural Resources Department
Developing Master Plan for 94 Acres of Town Property

The Town of Wake Forest is seeking qualified firms to create a Master Plan for 94 acres of Town property to be used for recreational purposes by the Parks, Recreation and Cultural Resources Department.

The scope of work shall consist of the following:

  • Property Analysis – Existing Conditions, Public & Staff Involvement, Mapping and Graphic Documentation, Strategic Plan and Implementation,

  • Request for Qualifications will be accepted until 2:00 p.m. on Monday, November 19, 2018 at the office of Randy Driver, Purchasing Manager, 234 Friendship Chapel Road, Wake Forest, NC 27587. Please send qualifications in a sealed envelope marked “RFQ # 19-001– Wake Forest Parks & Recreation 94 Acres Master Plan.”

Submissions shall include and be organized in this order:

Expertise

  • Demonstrate a strong background in creating master plans for property use, including projects that were completed within budget.

  • The ability to foresee and address potential issues and concerns.

  • The ability to work effectively with town staff, as well as citizens.

  • Project layout
     

Team Organization

  • Identify the project lead staff person and that person’s qualifications

  • Brief qualification and organization of other key team members involved with this project

  • Identify location of all members working on this project including current contact information for each member.


Project Vision

  • Statement of proposed project approach

  • Recommended scope of work

  • Timeline for completion


Legal History

Include a history of litigation and disputes including any pending litigation, arbitration, and disputes. Include results of past litigation, disputes, and arbitration.

All submittals will be scored by a panel, in each of the four categories stated above.

The RFQ shall list all subcontractor firms that will be used for this project.

List all minority participation in this project. The selected firm will be required to submit documentation validating the minority status and the dollar amount for this project.

The firm selected must provide a Certificate of Insurance with the following types and minimum amounts of insurance: General Liability – 1 million, Business Auto – 500 thousand, Employers Liability – 1million and Workers Compensation – Statutory Limits. The Town of Wake Forest must be listed as certificate holder on the Certificate of Insurance and as an additional insured under General Liability.

The Town of Wake Forest reserves the right to make the award as deemed in the best interest of the Town.
