- Criminal Justice Personal History Statement
Also known as Form F3, this application is used by the police department to adequately evaluate applicants for law enforcement and criminal justice certification. The questions are designed to ascertain whether the applicant meets the minimum standards for certification.

Any person wishing to report a violation may do so by completing the fom and returning it the Planning department anytime Monday – Friday between the hours of 8am – 5pm.

Application to rezone property within the Extraterritorial Jurisdiction of Wake Forest.
- Special Use Permit Application
Application for special uses as specified in the Town of Wake Forest Zoning Ordinance
- Plan Review Application
Application to the Development Services Division for residential and nonresidential development plans.
- Permit to Develop in the Special Flood Hazard Area (SFHA)
Town of Wake Forest, Development Services Division application for any construction proposed within the 100 year floodplain as regulated by the National Flood Insurance Program (NFIP) and the Town of Wake Forest Flood Damage Prevention Ordinance.
- Sign Permit Application
Application for the erection of a permanent sign within Wake Forest.
- Temporary Sign Permit Application
